Job summary

Molina Healthcare is seeking a care management professional to support LTSS activities and ensure coordinated member care across settings. You will collaborate with multidisciplinary teams to implement care plans, monitor progress, and address barriers affecting health outcomes.

Ideal candidates have LTSS experience, licensure as required by state contracts, and strong communication skills. Applicants should be prepared for travel and to engage with diverse member populations.

Qualifications

• At least 2 years of health care experience, including 1 year with disabilities/LTSS.
• Clinical licensure and/or certification required only if state contract mandates; active/unrestricted.
• Bachelor’s degree in a health care field may be required per state rules.
• Valid driver’s license, reliable transportation for travel.
• Strong knowledge of community resources.
• Excellent verbal and written communication; time management.

Responsibilities

• Completes comprehensive member assessments within regulated timelines; including in-person home visits if required.
• Facilitates waiver enrollment and disenrollment processes.
• Develops and implements care plans with members and care team.
• Monitors care plan effectiveness and documents interventions.
• Promotes integration of LTSS and behavioral health services.
• Assess medical necessity and authorizes waiver services.
• Travel 25-40% may be required depending on state/contractual needs.
