You will coordinate integrated member care, develop and monitor care plans, and ensure progress toward outcomes while aligning with organizational goals for quality and cost-effective care. The role involves waiver enrollment processes, care coordination across disciplines, and ongoing assessment of barriers to care.

Qualifications

• At least 2 years of health care experience with LTSS preferred.
• 1 year in care management or related medical/behavioral health setting.
• Clinical licensure/certification required only if state contract mandates.

Responsibilities

• Complete comprehensive member assessments within timelines, including home visits.
• Facilitate waiver enrollment/disenrollment processes.
• Develop and implement care plans with members and care networks.
• Monitor care plan effectiveness and adjust as needed.
• Promote service integration across behavioral health and LTSS.
• Assess medical necessity and authorize waiver services.
• Coordinate ICT meetings for service approval/denial.
• Educate and motivate members using motivational interviewing.
• Identify barriers and coordinate psycho/social/financial support.
• Travel 25-40% may be required based on state/contractual needs.
