Job summary

Old Colony Elder Services (OCES) in Massachusetts is expanding to the South Shore and seeks an OCP Advocate to empower people with disabilities to meet LTSS needs through assessment, person-centered care planning, advocacy and ICT participation.

You will identify appropriate LTSS, develop ICP components, facilitate community resources, and monitor services with the ICT. Frequent site travel and education of enrollees are part of the role.

Qualifications

• Knowledge of social work principles and aging.
• Ability to interview and assess personal information and draw conclusions.
• Ability to work harmoniously with consumers and staff.
• Ability to interpret complex regulations and procedures.
• Team-oriented with clear written and verbal communication.
• Bachelor's degree in social work or related field, LSW/LMHC preferred.
• Minimum two years in behavioral health or case management.

Responsibilities

• Assist in identifying LTSS needs and appropriate coordinators.
• Develop LTSS component of ICP and participate in ICT.
• Facilitate access to community LTSS and Personal Care Attendant services.
• Identify community alternatives to long-term institutional care.
• Monitor services per ICP and ICT requests.
• conduct initial and ongoing assessments of health and function.
• Advocate for enrollee needs and educate on LTSS to ICT and enrollee.
• Assess facility-based LTSS considerations for accessibility needs.
• Other duties as assigned.
