Old Colony Elder Services (OCES) is expanding services across the south shore, including Quincy and Weymouth, to support older adults and individuals with disabilities. The organization seeks a caregiver support professional to manage caseloads, assessments, and care planning, while fostering outreach and partnerships in the community.

The role emphasizes adaptability, strong communication, and collaboration with teams.

Qualifications

• At least 1 year of relevant experience in a community or elder service agency.
• Bachelor's degree in social work or related field.
• Excellent written and verbal skills.
• Proficient with Microsoft Office Suite or related software.
• Must have personal computer with secured internet access at home.

Responsabilités

• Maintain a caregiver caseload and complete assessments, care plans, and ongoing support.
• Facilitate or support caregiver groups and administer respite-related financial scholarships.
• Document services, outcomes, and program activity according to guidelines.
• Participate in outreach, educational events, and community presentations to promote OCES services.
• Build and maintain strong partnerships with community organizations, serving as a liaison to improve communication, collaboration, and the development of caregiver and wellness resources.
• Facilitate evidence-based and other community trainings.
• Follow business rules and program requirements for FCSP and Healthy Living initiatives.
• Complete required reporting and contribute to program evaluation.
• Demonstrate OCES core values.
