Job summary

OCES is expanding its services to include more towns along the south shore, assisting older adults and individuals with disabilities to stay active in their communities. The One Care Plan Advocate will lead LTSS assessments, develop person-centered care plans, and participate as a full ICT member.

The role emphasizes advocacy, coordination of LTSS resources, and education for enrollees and ICT, with travel between sites and a strong commitment to aging services.

Qualifications

• Knowledge of social work principles and aging processes.
• Ability to interview, assess, and make suitable recommendations.
• Ability to work in an interdisciplinary team and communicate clearly.

Responsibilities

• Assist in identifying appropriate LTSS Coordinator when needed.
• Contribute to LTSS component of ICP and ICT participation.
• Facilitate access to community LTSS and Personal Care Attendant services.
• Identify community-based alternatives to long-term institutional care.
• Monitor ICP-informed services and ICT guidance.
• Conduct initial and ongoing health and functional assessments.
• Advocate for enrollee needs and educate ICT and enrollee on LTSS.
