LTC Pharmacist Location: Worthington, OH 43085 Pay Rate: $66 per hour Schedule: Rotating 10-hour shifts Shift Options: 6:30 AM–4:30 PM, 9:00 AM–7:00 PM, 11:00 AM–9:00 PM, or 1:00 PM–11:00 PM Weekends: Every third weekend
Position Overview
We are seeking a Licensed Pharmacist to support long-term care (LTC) pharmacy operations. The pharmacist will oversee medication dispensing, ensure patient medication safety, provide clinical guidance, maintain accurate records, and supervise pharmacy staff.
Key Responsibilities
- Review, verify, prepare, and dispense prescription medications in accordance with applicable laws and regulations.
- Monitor patient medication compliance and identify potential medication misuse or overuse.
- Ensure prescribed medications are appropriate for each patient's needs.
- Maintain accurate, confidential patient records and pharmacy documentation.
- Counsel patients on medication use, dosage, storage, potential side effects, and drug interactions.
- Provide guidance on prescription and over-the-counter medications.
- Compound medications for internal and external use and prepare sterile solutions following appropriate procedures.
- Supervise pharmacy technicians, pharmacy clerks, and other pharmacy support staff.
- Maintain appropriate inventory levels of medications and medical supplies.
- Identify drug-related problems and communicate recommendations to physicians and other healthcare professionals.
- Collaborate with physicians, nurses, pharmacists, pharmacy interns, and other members of the healthcare team.
- Provide patient education on medication safety and medical devices, including blood pressure monitors and glucose meters.
- Support patient care services, including blood pressure monitoring, cholesterol management, and smoking cessation education, as applicable.
Qualifications
- Active Pharmacist license issued by the applicable state pharmacy board.
- Strong knowledge of medication safety, dispensing procedures, and pharmacy regulations.
- Excellent communication and patient counseling skills.
- Ability to identify medication-related issues and collaborate with healthcare professionals.
- Strong attention to detail and organizational skills.
- Ability to supervise pharmacy support staff and maintain accurate documentation.
- Availability to work rotating 10-hour shifts and every third weekend.
