LTC Pharmacist
Location: Greensburg, PA 15601
Pay Rate: $64.27/hour
Shift: 2nd Shift Schedule: Rotating schedule; days will change every 2 weeks
Must be available 7 days per week 3:00 PM - 11:00 PM
Every other weekend: Saturday & Sunday, 3:00 PM - 11:00 PM or 2:00 PM - 10:00 PM
2 holidays per year
Job Summary
We are seeking a licensed LTC Pharmacist to support medication dispensing, patient safety, pharmacy operations, and collaboration with healthcare professionals in a long-term care pharmacy setting.
Key Responsibilities
- Accurately review, prepare, and dispense prescription medications.
- Monitor medication compliance, overuse patterns, drug interactions, and potential drug-related problems.
- Ensure medications are appropriate for patients and comply with applicable laws and regulations.
- Maintain accurate and confidential patient records and pharmacy documentation.
- Counsel patients and healthcare professionals regarding medications, including dosage, administration, side effects, storage, and interactions.
- Provide consultation on prescription and non-prescription medications.
- Compound medications for internal and external use as needed.
- Prepare sterile solutions according to proper procedures and pharmacy standards.
- Supervise and provide direction to pharmacy technicians, clerks, interns, and other pharmacy staff.
- Maintain appropriate inventory of medications and medical supplies.
- Communicate effectively with physicians, nurses, other pharmacists, pharmacy interns, and healthcare professionals regarding patient medication needs.
- Educate patients on proper use of medical devices, including blood pressure monitors and glucose meters.
- Support patient care services and medication management initiatives.
- Ensure pharmacy operations are conducted safely, accurately, and in accordance with applicable regulations.
Qualifications
- Active Pharmacist license in the state of Pennsylvania.
- PharmD or equivalent pharmacy degree.
- Experience in long-term care, institutional, closed-door, or similar pharmacy settings preferred.
- Strong knowledge of medications, drug interactions, dosing, and pharmacy regulations.
- Excellent communication, organization, and attention to detail.
- Ability to work a rotating second-shift schedule, weekends, and required holidays.
- Compensation: $64.27/hour
