Position Overview
We are seeking a Licensed Pharmacist to join a long-term care (LTC) pharmacy team. This position is responsible for safely dispensing medications, supporting patient care, maintaining accurate records, and collaborating with healthcare professionals to ensure appropriate medication therapy.
Key Responsibilities
- Review, verify, prepare, and dispense prescription medications accurately and safely.
- Monitor patient medication compliance, medication usage, and potential overuse.
- Ensure medications are appropriate for patients and dispensed in accordance with applicable laws and regulations.
- Maintain accurate and confidential patient records and pharmacy files.
- Counsel patients regarding medications, including proper use, dosage, potential reactions, side effects, and drug interactions.
- Provide consultation regarding prescription and non-prescription medications, including storage and proper administration.
- Identify and communicate drug-related problems to physicians and other healthcare professionals.
- Compound medications for internal and external use as required.
- Prepare sterile solutions following appropriate procedures and standards.
- Supervise and provide guidance to pharmacy technicians, clerks, and other pharmacy staff.
- Maintain appropriate inventory levels of pharmaceutical and medical supplies.
- Collaborate with physicians, nurses, pharmacists, pharmacy interns, and other healthcare professionals to support patient care.
- Educate patients on the proper use of medical devices, including blood pressure monitors and glucose meters.
- Support pharmacy services such as smoking cessation, blood pressure monitoring, and cholesterol management when applicable.
- Perform all duties in accordance with state pharmacy regulations and professional standards.
Qualifications
- Active Wisconsin Registered Pharmacist (RPh) license required.
- Pharmacist experience in a long-term care, closed-door, retail, hospital, or similar pharmacy setting preferred.
- Strong knowledge of prescription medications, drug interactions, dosage, and medication safety.
- Excellent attention to detail and organizational skills.
- Strong communication and interpersonal skills.
- Ability to work effectively with pharmacists, pharmacy technicians, nurses, physicians, and other healthcare professionals.
