LTC Pharmacist
Location: Leesport, PA 19533 Pay: $65+ per hour Schedule: Monday–Friday, 9:00 AM–5:30 PM
Position Overview We are seeking an experienced Licensed Pharmacist to support operations within a long-term care pharmacy. This position is responsible for safely dispensing medications, providing medication consultation, maintaining accurate patient records, supporting pharmacy operations, and collaborating with healthcare professionals to ensure appropriate patient care.
Key Responsibilities
- Review, verify, and dispense prescription medications in accordance with applicable laws and pharmacy standards.
- Monitor patient medication compliance and identify potential overuse or medication-related concerns.
- Ensure medications prescribed to patients are appropriate and meet applicable safety and regulatory requirements.
- Maintain accurate and confidential patient records, pharmacy files, and documentation.
- Counsel patients regarding medications, including proper use, dosage, storage, potential side effects, and drug interactions.
- Provide consultation regarding prescription and over-the-counter medications.
- Educate patients on the proper use and handling of medical devices, including blood pressure monitors and glucose meters.
- Support patient services such as blood pressure monitoring, cholesterol management, and smoking cessation education, as applicable.
- Compound medications for internal or external use.
- Prepare sterile solutions while following appropriate procedures and safety standards.
- Supervise pharmacy technicians, clerks, and other pharmacy support staff.
- Maintain appropriate inventory levels of medications and medical supplies.
- Identify drug-related problems and communicate concerns or recommendations to physicians and other healthcare professionals.
- Collaborate with physicians, nurses, pharmacists, pharmacy interns, and other members of the healthcare team.
- Follow all applicable state and federal pharmacy laws, regulations, policies, and procedures.
Qualifications
- Active Pharmacist license in the state of Pennsylvania.
- Strong knowledge of medication safety, dispensing procedures, and pharmacy regulations.
- Excellent communication and patient counseling skills.
- Strong attention to detail and organizational abilities.
- Ability to identify medication-related problems and communicate effectively with healthcare professionals.
- Ability to work collaboratively with pharmacy and healthcare teams.
- Pharmacy or long-term care experience preferred.
Compensation & Schedule
- $65+ per hour
- Monday–Friday
- 9:00 AM–5:30 PM
