LTC Pharmacist
Location: Tulsa, OK 74146
Pay: $65+ per hour
Schedule: Monday-Friday, 9:00 AM-4:00 PM
Position Overview
We are seeking a licensed Long-Term Care (LTC) Pharmacist to support medication management and pharmacy operations for patients receiving home and long-term care services. This position is responsible for safely dispensing medications, evaluating drug therapies, providing medication consultation, maintaining accurate patient records, and collaborating with healthcare providers.
Responsibilities
- Accept, review, prepare, and dispense prescription medications accurately and safely.
- Monitor patient medication compliance, utilization, and potential overuse.
- Ensure medications are appropriate for each patient and comply with applicable laws and regulations.
- Evaluate and monitor drug therapies for home care patients.
- Maintain accurate, confidential patient records and pharmacy files.
- Counsel patients regarding medications, including proper administration, dosage, storage, side effects, and potential drug interactions.
- Answer patient questions regarding prescription and non-prescription medications.
- Compound medications for internal and external use as needed.
- Prepare sterile solutions according to established procedures and safety standards.
- Support compounding of infused drugs.
- Identify drug-related problems and communicate recommendations to physicians and other healthcare professionals.
- Collaborate with physicians, nurses, pharmacists, pharmacy technicians, and other members of the healthcare team.
- Supervise pharmacy technicians, clerks, interns, and other pharmacy staff as applicable.
- Maintain appropriate inventory levels of medications and medical supplies.
- Educate patients on the proper use and handling of medical devices, including blood pressure monitors and glucose meters.
- Provide medication-related services and patient education as needed.
- Maintain compliance with state pharmacy regulations and professional standards.
Required Qualifications
- Bachelor of Science in Pharmacy required; Doctor of Pharmacy (PharmD) preferred.
- Active Registered Pharmacist (RPh) license in the applicable state of practice.
- Strong knowledge of prescription medications, drug interactions, medication safety, and pharmacy regulations.
- Excellent verbal and written communication skills.
- Strong organizational and time-management skills.
- Ability to work effectively with patients, healthcare providers, and pharmacy staff.
Preferred Qualifications
- Experience compounding infused drugs.
- Experience evaluating and monitoring drug therapies for home care patients.
- Membership or participation in professional pharmacy societies and organizations.
- Long-term care, home infusion, specialty pharmacy, or home care experience.
