Job summary

Prestige Care Center of Plattsmouth in Nebraska is seeking an experienced MDS Coordinator (RN) to lead accurate MDS assessments, Care Area Assessments, and care plans in compliance with federal and state regulations.

The role requires thorough knowledge of Medicare, Medicaid, and managed care payers; timely communication with residents, families, and health care professionals; and coordination of Triple Check processes to ensure quality patient care.

Qualifications

• RN with MDS, CASA, and care planning experience.
• Solid regulatory knowledge of Medicare and Medicaid.
• Excellent communication and coordination with care teams.

Responsibilities

• Accurately complete MDS, Care Area Assessment, and Care Plans.
• Coordinate with residents, families, and other health care professionals for needed information.
• Communicate timely updates to the resident, family, and third‑party payers.
• Ensure Triple Check accuracy and timeliness in documentation.

MDS Coordinator (RN)

Accurate and thorough completion of the MDS, Care Area Assessment, and Care Plans, in accordance with current federal and state regulations and guidelines that govern the process.

Acts as an in-house case manager demonstrating detailed knowledge of residents' health status, critical thinking skills to develop appropriate care pathways, and timely communication of needed information to the resident, family, other health care professionals, and third-party payers.

Demonstrate an understanding of MDS requirements related to varied payers including Medicare, Managed Care, and Medicaid.

Responsible for timely and accurate completion of Triple Check.
