Job summary

Prestige Care Center of Plattsmouth is seeking an experienced MDS Coordinator (RN) to oversee MDS processes, CAAs, and care plans in a compliant manner. This role emphasizes in-house case management, interdisciplinary collaboration, and timely communication with residents, families, and payers.

The ideal candidate will have an RN license with strong MDS knowledge, regulatory understanding, and a commitment to high-quality, resident-centered care within our Plattsmouth facility.

Qualifications

• Must manage MDS processes accurately per federal and state regulations.
• Coordinate Care Area Assessments, Care Plans, and related documentation.
• Communicate with residents, families, and health care professionals to ensure optimal care.

Responsibilities

• Accurate and thorough completion of the MDS, Care Area Assessment, and Care Plans.
• Act as in-house case manager to develop appropriate care pathways.
• Timely communication of information to residents, family, other health care professionals, and third-party payers.
• Understand MDS requirements for Medicare, Managed Care, and Medicaid.
• Ensure Triple Check is completed timely and accurately.

MDS Coordinator (RN)

Accurate and thorough completion of the MDS, Care Area Assessment, and Care Plans, in accordance with current federal and state regulations and guidelines that govern the process.

Acts as an in-house case manager demonstrating detailed knowledge of residents' health status, critical thinking skills to develop appropriate care pathways, and timely communication of needed information to the resident, family, other health care professionals, and third-party payers.

Demonstrate an understanding of MDS requirements related to varied payers including Medicare, Managed Care, and Medicaid.

Responsible for timely and accurate completion of Triple Check.
