Job summary

Prestige Care Center of Plattsmouth is seeking an experienced MDS Coordinator (RN) to join our team. You will accurately complete the MDS, Care Area Assessments, and Care Plans in line with current federal and state regulations. Join a dedicated care team focused on health and healing.

In this in-house role, you will act as a case manager, communicating with residents, families, and health professionals, and ensuring timely Triple Check and payer-specific MDS requirements (Medicare, Medicaid).

Qualifications

• Accurate completion of the MDS, CARE Area Assessments and Care Plans per federal/state regulations
• In-house case management with care pathway planning and timely communication
• Understanding MDS requirements for Medicare, Managed Care and Medicaid
• Responsible for timely and accurate Triple Check completion

Responsibilities

• Accurate MDS completion and documentation to support resident care plans
• Coordinate assessments, care plans and related regulatory reporting
• Act as a case manager and liaison with residents, families and care teams
• Ensure compliance with payer requirements and timely communication

Accurate and thorough completion of the MDS, Care Area Assessment, and Care Plans, in accordance with current federal and state regulations and guidelines that govern the process.

Acts as an in-house case manager demonstrating detailed knowledge of residents' health status, critical thinking skills to develop appropriate care pathways, and timely communication of needed information to the resident, family, other health care professionals, and third-party payers.

Demonstrate an understanding of MDS requirements related to varied payers including Medicare, Managed Care, and Medicaid.

Responsible for timely and accurate completion of Triple Check.
