Job summary

Complete Care at Springbrook seeks a Licensed Social Worker to join our long-term care team for a full-time day shift. You will identify cognitive and psychosocial needs, document thoroughly, and educate residents and families on directives.

• Ability to build rapport with residents and peers.
• Prior long-term care or skilled nursing social services experience.
• LSW certificate is required.

Responsibilities

• Identifies cognitive impairments, mood problems and other psycho-social needs in residents.
• Maintain accurate, thorough, and timely documentation.
• Review Advanced Directives and provide education to residents and family members as needed.
• Help provide a smooth admission and discharge process for residents and family members.
• Other duties as assigned by Administrator.
