Creative Solutions In HealthCare seeks a dedicated Social Worker to support residents’ social and emotional well-being in a long-term care setting. The role emphasizes reliability, collaboration, and compassionate care, with responsibilities across assessments, care planning, and crisis support.

You will partner with residents, families, and a multidisciplinary team, lead orientation, discharge planning, and community referrals to ensure holistic care and smooth transitions.

Qualifications

• Requires a Bachelor’s Degree in Social Work or SW Designee certification.
• Must demonstrate effective communication and interpersonal skills.
• Experience in long-term care settings is preferred.
• Must be compassionate, organized, and focused on holistic resident well-being.

Responsibilities

• Develop detailed social histories and assessments for residents’ social and emotional needs.

We’re seeking a dedicated Social Worker to join our growing team! This role plays a vital part in supporting residents' social and emotional well-being. Reliability, trustworthiness, and consistency with attendance are essential. Your Impact as a Social Worker

In This Role, You Will

• Develop detailed social histories and assessments for residents, identifying medically related social and emotional needs.
• Create individualized care plans that support resident goals and emotional well-being.
• Participate in interdisciplinary care plan conferences and contribute to overall care planning.
• Provide individual and group support to residents and families during times of crisis or transition.
• Assist with new resident orientation and help residents and families adjust to the facility.
• Coordinate discharge planning for smooth resident transitions.
• Facilitate staff in-service training and educational programs related to social services.
• Build strong relationships with external partners (hospitals, doctors, churches, community agencies) to support referrals and ongoing care.
