Attain Behavioral Health Beachwood is seeking a compassionate Case Manager (LSW) to support clients in substance use disorder and behavioral health treatment programs in Ohio. • Licensed Social Worker (LSW) with a bachelor’s degree is required.
• Minimum one year of relevant behavioral health or case management experience.
• Strong crisis intervention and de-escalation skills.
• Ability to coordinate housing, employment, transportation, and healthcare resources.
• Excellent written and verbal communication; maintain professional boundaries.

Responsabilidades

• Providing case management services to clients in substance use disorder and behavioral health treatment programs.
• Assessing housing, employment, transportation, healthcare, mental health, legal, benefits, and education barriers.
• Developing and maintaining individualized case management plans based on client needs and goals.
• Connecting clients with housing, employment, medical, mental health, transportation, legal, and educational resources.
• Coordinating care with therapists, medical providers, and housing staff to ensure continuity of care.
• Utilizing crisis intervention and de-escalation techniques when clients are distressed.
• Maintaining professional, therapeutic interactions and confidential documentation.
• Completing all case management documentation within required agency timeframes.
• Participating in interdisciplinary meetings and trainings.
• Maintaining knowledge of local resources and regulatory requirements.
• Following policies, confidentiality, and ethical standards.
• Supporting KPI targets including documentation, engagement, and productivity.
• Completing other duties as assigned.
