Job summary

Wyandot Behavioral Health Network is seeking a compassionate Case Manager to serve adults with severe mental illness in the community. You will build trusting relationships, assess needs, and connect clients with housing, healthcare, and supportive services.

You will coordinate care with providers and families, advocate for clients, and maintain thorough documentation while supporting recovery and independence.

Qualifications

• Bachelor's degree in Social Work, Psychology, Rehabilitation, Human Services, or related field required.
• Valid driver's license and reliable transportation.
• Strong interpersonal, communication, organizational, and time management skills.
• Ability to work independently while collaborating with a multidisciplinary team.
• Basic computer proficiency and use of electronic health records.

Responsibilities

• Build meaningful relationships with individuals and families to support recovery.
• Conduct strengths-based assessments and develop personalized service plans.
• Coordinate care and services with providers, hospitals, families, and community partners.
• Connect clients with housing, healthcare, employment, transportation, and financial resources.
• Advocate with healthcare providers, courts, and government agencies.
