Job summary
MJHS is seeking a licensed practical nurse to perform hospice visits at patients’ homes or in nursing facilities across New York. You will document outcomes in the EMR and promptly report any changes to the supervising RN.
The role includes a backup on-call rotation 2–3 times monthly and 2–3 holidays annually. Applicants must graduate from an accredited LPN program with at least 1 year of Med/Surg experience and 1 year in Hospice or Oncology nursing.
Qualifications
- Graduate from an accredited Licensed Practical Nurse Program.
- Minimum 1 year LPN experience in Med/Surg and 1 year Hospice or Oncology Nursing.
- Good PC skills required. Driver's license and reliable transportation for visits (Manhattan exception).
Responsibilities
- Provide visits to hospice patients at home or in a Nursing Home.
- Document visits in the Electronic Medical Record and report changes to the RN.

Our groundbreaking hospice and palliative care programs offer a significant difference when dealing with a life-limiting condition. We offer a broad range of services in the community or facility-based to help patients and their families ease the burden of managing a life-limiting illness or end of life care.
In this position you will be responsible in providing visits to hospice patients in their home or in the Nursing home. You will document the visit in the Electronic Medical Record and report any changes to the patient's condition to the RN.
The schedule includes functioning in a backup On Call capacity 2-3 times per month and 2-3 holidays per year.
Graduate from an accredited Licensed Practical Nurse Program
Must have a minimum of one year experience as an LPN in Med/surg nursing and one year of Hospice or Oncology Nursing experience
Good PC skills a must
Drivers license and care required for all visits except in Manhattan
