Job summary
MJHS is seeking an LPN to provide compassionate hospice visits to patients in their homes or nursing facilities across New Rochelle, NY area. You will document each visit in the EMR and promptly report changes to the supervising RN.
The role includes on-call backup 2-3 times per month and 2-3 holidays per year, requiring a valid LPN license, at least one year of Med/Surg experience, and hospice/oncology exposure. Driver's license and reliable transportation are essential.
Qualifications
- Graduate from an accredited Licensed Practical Nurse Program.
- Minimum of one year LPN Med/Surg experience.
- One year Hospice or Oncology nursing experience.
- Good PC skills required.
- Driver's license; ability to visit homes (Manhattan exempt).
Responsibilities
- Provide visits to hospice patients in their home or in the Nursing home.
- Document visits in the Electronic Medical Record (EMR).
- Report any changes to the patient’s condition to the RN.

We offer a broad range of services in the community or facility-based to help patients and their families ease the burden of managing a life-limiting illness or end of life care.
In this position you will be responsible in providing visits to hospice patients in their home or in the Nursing home. You will document the visit in the Electronic Medical Record and report any changes to the patient's condition to the RN.
The schedule includes functioning in a backup On Call capacity 2-3 times per month and 2-3 holidays per year.
Graduate from an accredited Licensed Practical Nurse Program
Must have a minimum of one year experience as an LPN in Med/surg nursing and one year of Hospice or Oncology Nursing experience
Good PC skills a must
Drivers license and care required for all visits except in Manhattan
