Zusammenfassung

MJHS is seeking an LPN to provide hospice visits in patients' homes or nursing facilities. You will document visits in the EMR and promptly report changes to the supervising RN.

The role includes 2-3 on-call shifts per month and 2-3 holidays per year. Must be a graduate of an accredited LPN program with at least one year of hospice/oncology nursing experience and strong PC skills. A valid driver’s license is required for most visits, with exceptions in Manhattan.

Qualifikationen

• Minimum of one year experience as an LPN in Med/surg nursing and one year Hospice or Oncology Nursing experience.
• Must have good PC skills and valid driver's license.

Our groundbreaking hospice and palliative care programs offer a significant difference when dealing with a life-limiting condition. We offer a broad range of services in the community or facility-based to help patients and their families ease the burden of managing a life-limiting illness or end of life care.

In this position you will be responsible in providing visits to hospice patients in their home or in the Nursing home. You will document the visit in the Electronic Medical Record and report any changes to the patient's condition to the RN.

The schedule includes functioning in a backup On Call capacity 2-3 times per month and 2-3 holidays per year.
