MJHS Hospice seeks a Licensed Practical Nurse to provide compassionate care in patients' homes or nursing facilities in New York. The role emphasizes comfort, symptom management, and coordination with the care team.

You will document visits in the Electronic Medical Record, report changes to the RN, and participate in an on-call rotation about 2-3 times monthly with 2-3 holidays per year. A valid LPN license and good driving record are required.

Qualifikationen

• Graduate from an accredited Licensed Practical Nurse Program.
• Minimum of one year experience as an LPN in Med/surg nursing; hospice/oncology experience preferred.
• Strong PC skills and ability to document visits in EMR.
• Driver's license required; travel to visits (Manhattan excluded).

Our groundbreaking hospice and palliative care programs offer a significant difference when dealing with a life-limiting condition. We offer a broad range of services in the community or facility-based to help patients and their families ease the burden of managing a life-limiting illness or end of life care.

In this position you will be responsible in providing visits to hospice patients in their home or in the Nursing home. You will document the visit in the Electronic Medical Record and report any changes to the patient's condition to the RN.

The schedule includes functioning in a backup On Call capacity 2-3 times per month and 2-3 holidays per year.

Graduate from an accredited Licensed Practical Nurse Program

Must have a minimum of one year experience as an LPN in Med/surg nursing and one year of Hospice or Oncology Nursing experience

Good PC skills a must

Drivers license and care required for all visits except in Manhattan
