Job summary
MJHS Hospice is seeking a dedicated Licensed Practical Nurse to join our home-visit care team in New Jersey. You will provide visits to hospice patients at home or in nursing facilities, document in the EMR, and communicate changes to the RN.
The schedule includes on-call 2-3 times per month and 2-3 holidays per year; Manhattan visits are excluded. Must have 1 year Med/Surg LPN experience and hospice or oncology experience; strong PC skills and a valid driver's license are required.
Qualifications
- Graduate from an accredited LPN program.
- Minimum 1 year as an LPN in med/surg nursing.
- At least 1 year Hospice or Oncology nursing experience.
- Good PC skills required.

Our groundbreaking hospice and palliative care programs offer a significant difference when dealing with a life-limiting condition. We offer a broad range of services in the community or facility-based to help patients and their families ease the burden of managing a life-limiting illness or end of life care.
In this position you will be responsible in providing visits to hospice patients in their home or in the Nursing home. You will document the visit in the Electronic Medical Record and report any changes to the patient's condition to the RN.
The schedule includes functioning in a backup On Call capacity 2-3 times per month and 2-3 holidays per year.
Graduate from an accredited Licensed Practical Nurse Program
Must have a minimum of one year experience as an LPN in Med/surg nursing and one year of Hospice or Oncology Nursing experience
Good PC skills a must
Drivers license and care required for all visits except in Manhattan
