Job summary

MJHS is seeking a Licensed Practical Nurse to provide hospice care visits in patients' homes or nursing facilities in the Hempfield Township area. You will document visits in the EMR and report any changes to the attending RN.

The schedule includes backup on-call duties 2–3 times per month and 2–3 holidays annually. Ideal candidates graduate from an accredited LPN program with a minimum of one year in medical-surgical nursing and at least one year of hospice or oncology experience.

Qualifications

• Graduate from an accredited Licensed Practical Nurse Program.
• Minimum of one year experience as an LPN in Med/surg.
• One year hospice or oncology nursing experience.
• Good PC skills.
• Driver's license required for all visits (Manhattan exception).

Responsibilities

• Provide visits to hospice patients in home or Nursing home.
• Document visits in the Electronic Medical Record and report changes to the RN.
• Rotate to backup On Call 2–3 times per month and 2–3 holidays per year.

Our groundbreaking hospice and palliative care programs offer a significant difference when dealing with a life-limiting condition. We offer a broad range of services in the community or facility-based to help patients and their families ease the burden of managing a life-limiting illness or end of life care.

In this position you will be responsible in providing visits to hospice patients in their home or in the Nursing home. You will document the visit in the Electronic Medical Record and report any changes to the patient's condition to the RN.

The schedule includes functioning in a backup On Call capacity 2-3 times per month and 2-3 holidays per year.
