Job summary

Trilogy Health Services in Lowell, IN is seeking a Licensed Practical Nurse (LPN) to provide direct clinical care and supervise nursing assistants on the unit. The role requires daily resident assessments and timely communications with the charge nurse, physician, and DON.

Responsibilities include administering holistic care, monitoring medications and supplies, and completing charting and forms. Weekend shifts include a $5/hour incentive, with additional differentials for evenings or nights.

Qualifications

• Must have and maintain a current, valid state LPN license.
• Current, valid CPR certification required.

Responsibilities

• Visit residents daily to observe and report changes to the charge nurse/physician/DON.
• Identify problems and initiate lifesaving measures when physician is unavailable.
• Provide holistic care within LPN scope.
• Monitor medications, supplies, and equipment inventory.
• Complete medical forms, reports, evaluations, and charting as needed.
• Report occupational exposures per facility policy.
