Job summary

Trilogy Health Services in Lowell, IN is seeking a Licensed Practical Nurse (LPN) to provide direct clinical care and supervise nursing assistants on your unit. You will monitor residents daily, respond to changes, and ensure quality care within the LPN scope.

The role includes weekend shifts with a dedicated package: extra pay during weekend hours and additional differentials for evening or night weekend shifts, plus a robust benefits program for wellness, education, and retirement planning.

Qualifications

• Must have and maintain a current, valid state LPN license.
• Current, valid CPR certification required.

Responsibilities

• Visit residents on assigned unit daily to observe and evaluate condition and notify nurse/physician.
• Identify problems and emergencies and initiate lifesaving measures.
• Provide holistic care within LPN scope.
• Monitor medications, supplies, and equipment to ensure availability.
• Complete medical forms, reports, and charting as needed.
• Report occupational exposures per facility policy.
