Job summary

Trilogy Health Services in Logansport, IN, is seeking a Licensed Practical Nurse (LPN) to provide direct care to residents on the assigned unit and supervise nursing assistants. The role emphasizes compassionate, holistic care and timely communication with the charge nurse and physician.

Weekends include a dedicated weekend package: 2 shifts (8 or 12 hours) with an extra $5 per hour from Friday 6pm to Monday 7am, plus additional shift differentials for evenings or nights.

Qualifications

• Must hold a current, valid LPN license.
• Current CPR certification required.

Responsibilities

• Visit residents daily to observe physical and emotional status and notify nurse/physician of changes.
• Identify problems and emergencies and initiate lifesaving measures in the absence of a physician.
• Provide holistic care for residents within the LPN scope.
• Monitor inventory of medications, supplies and equipment to ensure adequate on-hand stock.
• Perform administrative duties such as completing medical forms, reports, evaluations, charting.
• Report occupational exposures per facility policies and procedures.
