Job summary

Trilogy Health Services is seeking a Licensed Practical Nurse (LPN) to provide direct, quality clinical care and supervise nursing assistants on assigned units at WoodBridge Health Campus in Logansport, IN. The role includes resident observation, care planning, and collaboration with the DON and physicians.

Weekend shifts are required, with incentives of $5.00/hour on eligible weekend hours and additional shift differentials for evenings or nights.

Qualifications

• Must hold a current, valid LPN license.
• CPR certification required.
• Weekend shifts are part of the role and require reliability.

Responsibilities

• Visit residents daily to monitor physical and emotional status and inform charge nurse/physician of changes.
• Identify problems and emergencies and initiate lifesaving measures in the absence of a physician.
• Provide holistic care within LPN scope.
• Monitor medications, supplies, and equipment to ensure adequate stock for residents.
• Complete medical forms, reports, evaluations, and charting as needed.
• Report occupational exposures per facility policies.
