Job summary

Valor Healthcare, Inc. is seeking a passionate Licensed Practical Nurse to join our Community Based Outpatient Clinic team in East Los Angeles. You will provide prescribed medical treatment and personal care services to patients, collaborating with the PACT team to develop the patient care plan.

You will support patient care and preventive services, document thoroughly in the EMR, and ensure confidentiality while delivering weekday, on-site care in a VA-adjacent clinic environment.

Qualifications

• Must hold a state-approved LPN/LVN diploma or certificate.
• Minimum one year of LPN/LVN experience in primary or ambulatory care; VA contract preferred.
• Current BLS (and ACLS as required by VA contract) certifications.
• Proficiency in written and spoken English and strong computer/EMR skills.

Responsibilities

• Assist with patient care, including routine nursing assessments and examinations.
• Administer medications as directed and document in the EMR.
• Operate ECG and other diagnostic equipment and conduct routine tests.
• Maintain patient confidentiality and provide patient/family education.
