Job summary

The Citadel of Kankakee is seeking a Licensed Practical Nurse (LPN) to join their team. The LPN will provide licensed care to residents, administer medications, and collaborate with the care team to ensure quality resident care.

Strong organizational and communication skills are essential, with a focus on safety and regulatory compliance. Applicants should hold an Illinois LPN license, CPR certification, and have 1 year of nursing experience (long-term care preferred).

Qualifications

• Must possess a current LPN license to practice in Illinois.
• CPR certification required.
• Experience in long-term care preferred.
• 1 year of nursing experience preferred.

Responsibilities

• Provide licensed nursing care to residents as ordered by physician and per standards.
• Record and report nursing data and shift handoffs.
• Maintain clean and sanitary work areas and equipment.
• Supervise CNAs on assigned unit to ensure compliant care.
• Complete resident assessments/observations and review care plans.
• Administer medications and treatments as ordered by physician.
• Document care notes accurately reflecting resident responses.
• Complete admission, transfer and discharge forms.
• Ensure safety and address hazards in resident environment.
• Communicate and make recommendations to the care team.
• Perform other duties as assigned.
