The Citadel of Bourbonnais is seeking a Licensed Practical Nurse (LPN) to join their team. As an LPN, you will care for a group of residents and assume responsibility for their care, through assessment, implementation, and evaluation of the plan of care.

Responsibilities include providing licensed care as ordered, performing assessments, monitoring vital signs, documenting notes, ensuring safety, communicating with families and physicians, and adhering to infection control and HIPAA policies.

Qualifikationen

• Nursing degree or diploma from an accredited College or University.
• Possess a current license to practice in the State as a Licensed Practical Nurse.
• 1 year of experience preferred.
• Long-Term care experience preferred.

Aufgaben

• Provides licensed care to assigned residents as ordered by physician and in accordance with facility, federal, state and local standards, guidelines and regulations.
• Provides required resident assessments/observations and interventions, reviewing care plans to ensure appropriate care.
• Takes and records residents blood pressure and vital signs, administers catheter care, administers IV therapy to residents as ordered within license limitations, and provides respiratory care.
• Charts nurse’s notes reflecting the care provided and resident response.
• Ensures environment is free from accidents and hazards and addresses potential safety risks.
• Monitors and directs non-licensed ancillary services to meet professional standards and maintain high-quality care.
• Communicates information to residents, families, physicians, guardians, and facility personnel as required by policy and guidelines.
• Ensures care plans are implemented and maintained; makes recommendations for changes to the IDT.
• Follows infection control standards and HIPAA policies.
• All other duties as assigned.
