Job summary

The Citadel Healthcare is actively seeking a Licensed Practical Nurse (LPN) in Illinois to join their caregiving team. The LPN will oversee a group of residents for an assigned shift, administer medications, perform assessments, and ensure adherence to federal and state standards.

Key responsibilities include shift reports, care plan reviews, CNA supervision, and documentation of resident responses. Strong communication, clinical skills, and flexibility are required for this role.

Qualifications

• Current CPR Certification required.
• Possess a current license to practice in the State as a Licensed Practical Nurse.
• 1 year of experience preferred.
• Long-Term care experience preferred.

Responsibilities

• Provides licensed care to residents as ordered by physician and in accordance with standards.
• Maintains nursing reports at shift change and ensures a clean, sanitary area.
• Directs and supervises CNAs to ensure care aligns with standards.
• Performs resident assessments, reviews care plans, and administers medications.
• Charts nursing notes reflecting care and resident responses.
• Receives and transcribes physician orders to MAR and care plans.
• Manages adverse events and safety risks in the resident environment.
