Citadel Health Care in Freeport, IL is seeking a Licensed Practical Nurse (LPN) to join our team on a full-time basis. The LPN assumes responsibility for a group of residents during a shift, providing licensed care and collaborating with physicians to follow orders.

Qualifications

• Current CPR Certification required.
• Possess a current license to practice in the State as an LPN.
• 1 year of experience preferred.
• Long-Term care experience preferred.

Responsibilities

• Provide licensed care to residents as ordered by physician and in accordance with standards.
• Provide and receive nursing reports at shift changes.
• Maintain clean and sanitary work areas and rooms.
• Direct and supervise CNAs to ensure care adheres to standards.
• Perform resident assessments/observations and review care plans.
• Take vitals and administer medications as ordered.
• Transcribe physician orders to MAR and charts.
• Complete forms on admission, transfer and discharge.
• Ensure resident safety and address hazards.
• Document nurse notes reflecting care and resident response.
