Chandler Creek Post Acute in Greer, SC is seeking an experienced LPN Unit Manager to lead the nursing team and ensure exceptional resident care. This role collaborates with the Director of Nursing and the interdisciplinary team to oversee daily clinical operations and staff development.

Qualifications

Current, unrestricted LPN license in South Carolina.
Graduate of an accredited nursing program.
Current CPR certification.
Leadership or management experience in skilled nursing or long-term care preferred.
Experience with PointClickCare (PCC) preferred.
Strong clinical assessment, communication, and leadership skills.
Ability to lead by example and foster a positive team culture.

Responsibilities

Assist the Director of Nursing in planning and directing day-to-day nursing operations.
Oversee clinical care delivery within the assigned unit.
Supervise, mentor, and support nursing staff to ensure quality resident care.
Monitor resident conditions and ensure timely interventions.
Participate in care planning meetings and interdisciplinary team discussions.
Ensure accurate clinical documentation and compliance with policies.
Assist with staff scheduling, training, coaching, and performance management.
Support regulatory compliance and quality assurance initiatives.
Serve as a resource for residents, families, and staff regarding care concerns.
Promote a positive work environment focused on teamwork and resident-centered care.
