Qualifikationen

• LPN license required with active, unrestricted state license.
• CPR certification required.
• Prior supervisory or leadership experience in skilled nursing or long-term care preferred.
• Experience with PointClickCare (PCC) software preferred.

Aufgaben

• Oversee daily clinical operations of an assigned unit.
• Supervise and support CNAs and nursing staff.
• Monitor resident conditions and ensure timely follow-up on changes.
• Conduct rounds and address resident, family, and staff concerns.
• Assist with admissions, transfers, and discharges.
• Maintain accurate clinical documentation.
• Participate in care plan meetings and interdisciplinary conferences.
• Ensure compliance with federal, state, and facility regulations.
• Support survey readiness and quality improvement efforts.
• Collaborate with physicians, therapists, and department leaders to achieve positive resident outcomes.
