Job summary

Fountain Inn Post Acute is seeking a Licensed Practical Nurse (LPN) to provide direct nursing care to residents at our 66-bed facility in Fountain Inn, SC. You will work with the care team to support residents’ health and well-being, administering medications and documenting care per individualized plans.

Qualifications

• Active LPN license in South Carolina (required).
• Current CPR certification required.
• Experience with PointClickCare (PCC) preferred.
• Strong clinical, communication, and teamwork skills.

Responsibilities

• Deliver direct patient care in accordance with individualized care plans.
• Administer medications and treatments as prescribed.
• Monitor resident conditions and report changes to the clinical team.
• Support and supervise CNAs to ensure high-quality care delivery.
• Maintain accurate and timely clinical documentation.
• Promote a safe, clean and respectful environment.
• Collaborate with interdisciplinary team members.
