Job summary

MJHS Hospice invites a dedicated Licensed Practical Nurse to provide home and nursing home visits, documenting care in the EMR and communicating changes to the RN. The role includes 2-3 on-call shifts per month and 2-3 holidays annually, requiring compassionate end-of-life support.

Applicants must graduate from an accredited LPN program with at least one year of Med/Surg experience and one year in Hospice or Oncology nursing, plus strong PC skills and a valid driver's license for all visits

Our groundbreaking hospice and palliative care programs offer a significant difference when dealing with a life-limiting condition. We offer a broad range of services in the community or facility-based to help patients and their families ease the burden of managing a life-limiting illness or end of life care.

In this position you will be responsible in providing visits to hospice patients in their home or in the Nursing home. You will document the visit in the Electronic Medical Record and report any changes to the patient's condition to the RN.

The schedule includes functioning in a backup On Call capacity 2-3 times per month and 2-3 holidays per year.

Drivers license and care required for all visits except in Manhattan
