Job summary

MJHS is seeking a Licensed Practical Nurse to provide hospice visits in patients’ homes or nursing facilities. You will document visits in the EMR and promptly report changes to the supervising RN.

The role includes a backup On Call capacity 2-3 times per month and 2-3 holidays per year. Requires LPN from an accredited program with at least 1 year in Med/Surg and hospice/oncology experience, plus good PC skills and a valid driver’s license.

Qualifications

• Graduate from an accredited Licensed Practical Nurse Program.
• Minimum of one year experience as an LPN in Med/surg nursing and one year of Hospice or Oncology Nursing experience.
• Good PC skills required.
• Driver's license and ability to travel to visits (Manhattan not applicable).

Responsibilities

• Provide visits to hospice patients in their home or in Nursing homes.
• Document visits in the Electronic Medical Record (EMR).
• Report any changes in patient condition to the RN.
• Participate in On Call backup 2-3 times per month and 2-3 holidays per year.

Our groundbreaking hospice and palliative care programs offer a significant difference when dealing with a life-limiting condition. We offer a broad range of services in the community or facility-based to help patients and their families ease the burden of managing a life-limiting illness or end of life care.

In this position you will be responsible in providing visits to hospice patients in their home or in the Nursing home. You will document the visit in the Electronic Medical Record and report any changes to the patient's condition to the RN.

The schedule includes functioning in a backup On Call capacity 2-3 times per month and 2-3 holidays per year.
