Qualifications
- Minnesota LPN license active and in good standing.
- Graduation from an accredited practical nursing program.
- CPR/BLS certification required or maintained.
- Long-term care or skilled nursing experience preferred.
- Strong clinical, communication and teamwork skills.
Responsibilities
- Provide direct nursing care per care plans.
- Administer medications and treatments as prescribed.
- Monitor residents and report changes to RN/physician.
- Document nursing interventions accurately and promptly.
- Assist with admissions, discharges, and transfers.
- Collaborate with CNAs and staff to ensure quality care.
- Maintain a clean, safe environment for residents.
- Educate residents and families on health and care needs.
