Job summary

Monarch Healthcare Management in Minnesota is seeking a Licensed Practical Nurse (LPN) to deliver high-quality direct care to residents under RN supervision, aligned with Minnesota BON standards and residents' rights.

The LPN will participate in care planning, monitor resident health data, delegate tasks to unlicensed staff, and support residents' physical, mental, and psychosocial well-being while contributing to QAPI initiatives.

Qualifications

• Completion of a professional nursing education program approved by the Minnesota Board of Nursing.
• Current Minnesota LPN license.
• Current CPR certification.
• Experience in skilled nursing and/or assisted living facilities.

Responsibilities

• Provide residents with direct routine care under supervision of RN and in accordance with BON standards.
• Participate in development and modification of the resident Plan of Care; implement delegated interventions.
• Collect health data, report changes to the RN, and monitor interventions ongoingly.
• Assign and monitor nursing tasks to unlicensed staff, clarifying expectations and addressing behaviors.
