Job summary

Live Well Homecare is seeking an Licensed Practical Nurse (LPN) Medication Nurse to administer medications and treatments per physician orders in a homecare setting. You will document in MAR/TAR, monitor residents, report adverse reactions, and ensure safety and regulatory compliance.

The role emphasizes accurate assessment, timely communication, and meticulous record-keeping to support high-quality home-based care for seniors.

Qualifications

• Active LPN license in good standing.
• CPR and First Aid certification.
• Strong assessment and documentation skills.

Responsibilities

• Administer medications and treatments following physician orders.
• Accurately document in MAR/TAR and maintain medication carts.
• Monitor symptoms and report adverse reactions to the care team.
• Transcribe physician orders and observe changes in residents’ status.
• Ensure compliance with safety standards and complete narcotic counts as required.
