Job summary

LiveWell Homecare Agency, LLC is seeking a Licensed Practical Nurse (LPN) Medication Nurse to support residents in a senior living setting, focusing on safe medication administration, health monitoring, and accurate clinical documentation.

Responsibilities include administering medications per physician orders, documenting in MAR/TAR, monitoring residents for changes, reporting adverse events, and ensuring regulatory compliance to safeguard resident safety.

Qualifications

• CPR and First Aid certification.
• Strong assessment and documentation skills.
• Preferred experience in senior care or medication administration.

Responsibilities

• Administer medications and treatments according to physician orders, adhering to the Six Rights of Medication Administration
• Document all procedures accurately in MAR/TAR in real time
• Monitor residents’ symptoms, administer medications for relief, and report any refusals or adverse reactions
• Conduct SHUs, transcribe physician orders, and support care plan updates
• Observe and report physical, emotional, or cognitive changes in residents
• Maintain medication carts, complete narcotic counts, and ensure compliance with regulations
