Job summary

LiveWell Homecare Agency, LLC is seeking a dedicated Licensed Practical Nurse (LPN) Medication Nurse to provide licensed nursing support in a senior living community. The role focuses on safe medication administration, health monitoring, and precise clinical documentation to ensure resident safety.

Responsibilities include administering medications per physician orders, documenting in MAR/TAR in real time, monitoring residents for symptoms, reporting adverse reactions, and supporting care plan

Qualifications

• CPR and First Aid certification required.
• Strong assessment and documentation skills.
• Experience in senior care or medication administration preferred.

Responsibilities

• Administer medications and treatments following physician orders and the Six Rights of Medication Administration
• Document accurately in MAR/TAR in real-time
• Monitor resident symptoms, report adverse reactions, and support care plan updates
• Conduct SHUs and transcribe physician orders
• Observe and report physical, emotional, or cognitive changes
• Maintain medication carts, complete narcotic counts, and ensure regulatory compliance
