Job summary

Aspire Senior Living Pleasant Hill in Missouri is seeking a dedicated LPN/RN MDS Coordinator to join our team. The role centers on coordinating the MDS process, developing individualized care plans, and maintaining detailed records in a warm, resident-focused setting.

You will work with physicians, nurses, and other healthcare professionals to ensure high-quality care, while providing excellent service to residents and families.

Responsibilities

• Coordinating the MDS process from start to finish, ensuring compliance with federal regulations and company protocols
• Assessing resident needs and developing individualized care plans
• Collaborating with interdisciplinary teams, including physicians, nurses, and other healthcare professionals
• Maintaining accurate and detailed records, including MDS assessments, care plans, and progress notes
• Providing exceptional customer service and support to residents, families, and team members
