Job summary

Collierville Nursing and Rehabilitation in Collierville, TN is seeking a detail-driven MDS Coordinator (LPN or RN) to lead MDS assessments, care planning, and regulatory compliance.

You will work with an interdisciplinary team to ensure accurate MDS/RAI formulation, review documentation, and support chart audits while providing clinical guidance.

This role requires MDS 3.0 experience, knowledge of Medicaid/Medicare, and long-term care background.
