Job summary

Cordova Wellness & Rehabilitation Center is seeking a MDS Coordinator (LPN or RN) to join our care team in Cordova, TN. You will manage the MDS/RAI processes and work with an interdisciplinary team to ensure accurate documentation and care planning.

The role requires active Tennessee LPN/RN licensure, PDPM/CMI experience, and RAC preferred. This is a full-time position with a supportive team and comprehensive benefits.

Qualifications

• Active LPN or RN license for Tennessee.
• 2-5 years experience with PDPM, CMI, and Quality Measures.
• RAC Certification preferred.

Responsibilities

• Attend weekly education meetings to stay updated on MDS changes.
• Coordinate the facility's RAI process per state and federal guidelines.
• Accurately complete all MDS assessments and supporting documentation.
• Evaluate residents' care plans and audit medical records with the interdisciplinary team.
• Perform tasks as assigned by Regional MDS Consultants, Administrator, and Director of Nursing.
• Maintain confidentiality of protected health information.

MDS Coordinator Position Summary

As a MDS Coordinator (LPN or RN) at Cordova Wellness & Rehab, you'll be an essential part of our residents' care. Responsibilities include:

• Attend weekly educations meetings to stay updated on MDS changes.
• Coordinate the facility’s Resident Assessment Instrument (RAI) process in accordance with state and federal guidelines.
• Accurately complete all MDS assessments and any supporting assessments or clinical documentation.
• Evaluation of resident’s comprehensive plan of care, auditing medical records for supporting documentation, collaborating with the interdisciplinary team.
• Perform any other additional tasks as assigned by the Regional MDS Consultants, Administrator, and Director of Nursing.
• Maintain confidentiality of protected health information, including verbal, written and electronic communications.

MDS Coordinator Requirements

• Active LPN or RN license for the state of Tennessee
• 2-5 Years Experience with PDPM, CMI, and Quality Measures
• RAC Certification preferred
