Qualifications

• Massachusetts-licensed RN or LPN required.
• Medication administration proficiency.
• Experience with EMR PCC preferred.
• Knowledge of memory care and sub-acute care.
• Strong interpersonal and communication skills.

Responsibilities

• Assess resident health needs and develop nursing care plans.
• Administer medications and treatments as prescribed and monitor responses.
• Maintain accurate documentation in PCC EMR and ensure HIPAA compliance.
• Collaborate with multidisciplinary teams to deliver comprehensive care.
• Educate residents and families on health conditions and discharge plans.
