Qualifications

• Licensed nurse in MA with active nursing license.
• Proficient in administering medications and treatments.
• Experience with PCC electronic medical records (EMR).
• Familiarity with MDS assessments for long-term care.
• Strong case management and teamwork skills.

Responsibilities

• Assess resident health problems and develop nursing care plans.
• Administer medications and treatments as prescribed by physicians.
• Monitor resident progress and respond to changes in condition.
• Collaborate with multidisciplinary teams to provide comprehensive care.
• Educate residents and families regarding health conditions, treatment plans, and discharge.
• Maintain accurate resident records and HIPAA-compliant documentation.
• Participate in case management and support memory care needs.
