Job summary

Brethren Village Retirement Community in Lititz, PA seeks a qualified LPN or RN Clinical Care Coordinator for our Personal Care and Memory Support areas. The role collaborates with the Director of Nursing, leads staff, and ensures high-quality resident care, including updating care plans and providing direct care when required.

The candidate will have opportunities for ongoing clinical skills training, mentoring, and professional development under Nursing leadership.

Qualifications

• PA LPN or RN credentials (active)
• Current CPR and First Aid certification
• At least 2 years of nursing experience in personal care, skilled care, or acute care
• Experience in leadership, supervisory, or mentoring roles
• Experience with patients with dementia and HIPAA knowledge

Responsibilities

• Monitor and manage Residents' care needs and update care plans
• Provide leadership, oversight, and guidance for assigned staff
• Provide direct patient care as needed and collaborate with the Director of Nursing
