Job summary

Tree of Life Rehabilitation seeks an RN/LPN Supervisor to oversee care delivery, supervise RN/LPNs, CNAs, and Medication Aids, and ensure timely resident assessments in a faith-based long-term care setting.

The role requires an active RN or LPN license in good standing in Wisconsin. Prior skilled nursing experience is preferred; we will invest in candidates with the right mindset. Benefits include a comprehensive package and opportunities for advancement.

Qualifications

• Active RN or LPN license in good standing in the state of practice.
• Experience in a skilled nursing or long-term care setting is preferred but not required, we’ll invest in the right heart and mindset.

Responsibilities

• Supervise delivery medications and treatments along with supervision of RN/LPN Charge Nurses, CNAs, and Medication Aids where applicable
• Completion of required admission and routine resident assessments
• Monitor resident conditions and respond quickly to changes, ensuring safety, comfort, and continuity of care.
• Support activities of daily living
• Maintain clear, timely documentation and communicate effectively with interdisciplinary teams and families
• Adhere to infection control and safety standards to protect residents and teammates
• Other duties as assigned
