Job summary

CommWell Health in Tar Heel, NC is hiring a licensed practical nurse (LPN) or RMA professional to join our rural-focused care team. You will deliver high-quality, patient-centered care across the lifespan and support care coordination with providers using our EHR.

We offer a supportive environment, flexible scheduling, training, and opportunities for growth. Qualifications include active NC LPN or RMA license, graduate of an accredited program, CPR/BLS, and a valid driver's license.

Qualifications

  • Active LPN or RMA license/certification in North Carolina in good standing.
  • Graduate from an accredited nursing program with a solid academic record.
  • High school diploma or GED required.
  • Current CPR/BLS certification.
  • Valid driver’s license and reliable transportation.

Responsibilities

  • Deliver compassionate, high-quality care to patients across the lifespan.
  • Partner with providers and care teams to ensure seamless treatment.
  • Support patients and families in understanding health needs and next steps.
  • Document care accurately using the EHR system.
  • Participate in care huddles and team-based quality improvement initiatives.
  • Serve as a key member of the Patient-Centered Health Home model.

What We’re Looking For

Required:

  • LPN / RMA License OR Certification, active and in good standing with the State of North Carolina
  • Graduate of an accredited program
  • Highschool graduated/GED
  • Current CPR/BLS certification
  • Valid Driver’s License

Preferred:

  • One (1) year of clinical experience in a primary care, community health, or outpatient setting
  • Experience with underserved or rural populations
  • Familiarity with team-based care or PCMH/PCHH models
